Lost certificates

Important information

If you have lost your certificate or certifying statement of results, you can request a replacement by completing and returning the appropriate application form. Please ensure you complete the correct form, as the procedure is different depending on the type of qualification.

Please remember, without an application, we won't be able to confirm whether we hold your results.

Replacing lost certificates for 14-19 qualifications

14-19 qualifications include: AEA, Asset Languages, CEE, CSE, Entry Level Certificate, FSMQ, GCE A/AS Level, GCE O Level, GCSE, GNVQ (post 2001), STEP and VCE.

OCR cannot supply replacement certificates to candidates for 14-19 qualifications. If you lose your certificate, you must apply for a certifying statement of results. This is an official copy of the final exam results held by OCR and will be accepted by universities, colleges and employers as verification of results.

To obtain a replacement certificate for Certificates of Professional Competence (CPC) National or International full awards, please complete and return the application form below.

Replacing lost certificates for vocational qualifications

Vocational qualifications include: Basic Skills, CBLC, CLAiT, Functional Skills, GNVQ completed prior to June 2001, Key Skills, NVQ, OCR Nationals, and former RSA examinations.

Verifying the results of an individual

If third party organisations wish to verify the results of an individual, they should contact the OCR Customer Contact Centre for an application form. Please note that learners cannot apply for this service (although they will be asked to sign the application form).
