      Updated candidate declaration form for Design and Technology 01 October 2026

      All the JCQ awarding bodies have introduced updated candidate and teacher authentication declarations for qualifications with non-exam assessment where authentication is required.

      We’ve incorporated these declarations into an updated Cambridge OCR qualification-specific candidate declaration form for:

      • GCSE Design and Technology (J310)
      • AS Level Design and Technology (H004-H006)
      • A Level Design and Technology (H404-H406)

      You must use this form for non-exam assessment completed from September 2026.

      Centres must make sure all candidates have read the notice to candidates section of the form before beginning their work. Candidates should only sign the declaration once their work has been completed and before it is submitted to their teacher.

      The completed forms must be submitted with the centre sample as part of the normal submission requirements. Any portfolios not requested as part of the sample must be retained until all reviews of results, malpractice investigations and appeals have been resolved.

      As the GCSE, AS and A Level Design and Technology qualifications have their own authentication form, you are not required to also use the new generic candidate and teacher authentication form, described in our subject update of 22 September 2026.

      Accessing the new form

      The updated GCSE, AS and A Level Design and Technology candidate declaration form is available from the Assessment > Forms section on Teach Cambridge and from the submission requirements page on our website.
